NTSB: Lack of oversight, sleep-deprived driver led to 2016 Calif. bus crash

A motorcoach operated by Fresno-based Autobuses Coordinados USA Inc., traveling from Los Angeles to Modesto on State Route 99, drifted out of its travel lanes, striking a barrier system and a highway signpost shortly after 3 a.m., Aug. 2, 2016, near Livingston, Calif.

Chicago train crash caused by operator fatigue, union says

A union representative reported in a news conference that the woman said she had recently worked extensive overtime recently and was "extremely tired" at the time of the accident. NTSB says the train was traveling at a normal speed of 25 mph before it crashed.

Dutch bus drivers to test fatigue warning system

Royal Beuk BV, a Dutch luxury bus company, is outfitting 20 vehicles with a system that is designed to detect if a driver is becoming drowsy. The technology, created by Seeing Machines, uses infrared light and a camera to track eye movements.

Closure of Fung Wah sends clear message

At the end of February, the FMCSA declared the notorious Fung Wah Bus Transportation Inc. to be an imminent hazard and sent the company an out-of-service order, which ordered Fung Wah to immediately provide its entire fleet of 28 motorcoaches for thorough and detailed safety inspections by qualified inspectors. The company was then shut down after not cooperating with FMCSA safety investigators and blocked further access to company safety records.
