NTSB: D.C. Metro’s safety problems preceded crash

WASHINGTON, D.C. — The Associated Press reports that on Tuesday a federal official claimed D.C. Metro’s safety problems extended past a signaling system failure, which was determined to be the cause of the 2009 Metrorail crash that killed nine people.
