The incidents happened within six weeks of each other after the end of the woman’s trip on the Toledo Area Regional Paratransit Service. Although the passenger was brought home safely both times, one incident resulted in an investigation.

The investigative group focusing on the track has learned from inspection reports that in April, maintenance work was done in the area of the derailment of the eastbound train. Records revealed that a joint bar, used to join two sections of rail together, was cracked and that it was repaired by Metro-North personnel.

An investigation culminated in the dismissal of eight TTC transit enforcement officers, five of whom were arrested and face charges of attempt to obstruct justice and fabricate evidence. The five officers allegedly submitted false offense tickets to individuals of no fixed address for panhandling on TTC property, loitering or trespassing.

When asked at a hearing why the agency didn’t move hundreds of railcars and engines before superstorm Sandy struck, executive director Jim Weinstein defended the decision. He said it was based on the fact that its yards had never flooded in NJ Transit’s nearly 30-year existence.

Report said the agency launched a probe into its Superstorm Sandy preparations, looking primarily at the decision to store trains in areas that ended up getting flooded. The potential damage could cost millions and may have been preventable.

On May 31, 2011, the motorcoach, operated by Sky Express Inc., was traveling northbound on I-95 in the vicinity of milepost 103 in Caroline County, Va., when it departed the right side of the highway and rolled over.
