NTSB: Jumper wire install cause of Miami people mover crash

Determined that the probable cause of the Nov. 28, 2008, accident at Miami International Airport, involving an automated people mover train, was the installation of a “jumper wire” that prevented the overspeed/overshoot system from activating to stop the train when the crystal within the primary program stop module failed.

The fatal accident rate for curbside carriers from January 2005 to March 2011 was 7 times that of conventional bus operations: 1.4 fatal accidents per 100 vehicles for curbside carriers compared with 0.2 fatal accidents per 100 vehicles for conventional scheduled carriers.
